I second the vote for Red Bank in Tinton Falls, NJ.  I had to take one of my boys there for an emergency on Father's Day, and they were very compassionate...I was there for hours, and in the end they couldn't save him, but they gave me his body in a little box, and comforted me when I bawled my eyes out.

I'm not sure they treat exotics often, but I had to take one of my poor girls (RIP) to get stitches after a little scuffle and they did a wonderful job. Even if it's something uncommon, they'll take the time to figure it out, and give your baby great care.

The staff was very welcoming and compassionate when I had to rush in at 2am to have my beloved Chester pts. They took care of him and the vet even teared up when she told me what a sweet and patient guy he was with her throughout the whole process. All three staff members (front desk, vet, and tech) took my situation very seriously and handled Chester with the utmost respect. They even sent me a sympathy card even thought it was the first time I had been to that location.

Dr. Mikelson is a great vet who will take all the time you need to answer questions and never rush you out the door. He truly cares about each animal who walks in the doors. I think he's only there Tuesdays and Thursdays or something like that. He works at two sites.

They're not open 24 hours a day, but do have hours every day of the week.  They have 3 vets, one of whom is the exotics vet (Dr. Steeves).  She did a wonderful job with our parakeet recently, who needed a leg amputated, so I took one of my sick foster girls there.  Their facility is well-equipped and, unlike my regular vet, they had the medications I needed in formulations that are yummy for a rat.  More expensive than my regular vet, so I don't think I'd go there for anything routine, but I did have a positive experience with them.

Animal and Bird Health in Cherry Hill, NJ is a good vet. They can be a bit expensive, but knowledgeable.  Respectful of rats as pets. Dr. Boutette is who we see. On an emergency visit I have seen Dr. Dazen (he was on call that Sunday- doesn't usually see rats) and he was so patient, very knowledgeable, and wouldn't euthanize until we sat down and made sure we had discussed every treatment option to make sure it was tried.

Comments: Although they are not a 24 hour clinic, if you call their emergency number they will have a vet come in at any time to provide emergency care.

I DO NOT SUGGEST ACCESS (11536 Lake City Way Northeast  Seattle, WA 98125 (206) 364-1660) although they claim to have experience with small animals, I took a rat there when I was a very inexperienced owner and they told me to NEBULIZE BAYTRIL! And did not give me any nebulizing medications, or meds other than Baytril (which I was directed to use incorrectly). The vet led me to become very confused, over charged me for the very little care he provided, and ultimately ended with the death of my rat.

In The Bay Area California, if anyone is out here, The Boulevard Pet Hospital in Castro Valley CA are amazing.  I had to take all 11 of my babies in there the other day (7 five day olds, 2 three month olds, my one year old mamma, and approx 1.5yo daddy) they saw all of them for a very reasonable fee and the meds were very cost effective as well.  Their Vet is amazing (I don't remember his name) And Robert the Vet tech is wonderful as well, he has had me calling him every day to give him updates on the 3month olds as they have colds, and has been very patient with me in regards to dosing instructions.  Their number is 510-538-1302.  I -highly- recommend them!

VCA old Marple Animal Hospital has a few vets who will see small animals - but I think in an emergency any of the vets there will.

Midwest exotic animal center/emergency animal hospital in Elmwood Park, IL sees ratties often. They see rats daily  They're located just west of Chicago on North Ave. Luckily they're a 24 hr facility.  Dr. Clarkson is really nice as is Dr. Paliakoff. They are very caring and kind and understand our love for ratties.

For anyone around Knoxville, TN:

UT Avian and Exotics Clinic is open monday-saturday on regular appointment basis and Sundays are open for emergencies and will take you without appointment any day if you have an emergency.
Be prepared for the fee to just see them if it's an emergency, it's $155, but they are willing to work with people about payment plans as they understand that it's not easy when you have a sick animal, it adds up quick.

All vets on staff can treat rats and exotics. I have had to use their services for emergency surgery (internal spay stitches reopened due to hyperactive young lady rat) and euthanasia and they have been very caring, supportive and professional. I absolutely recommend them. They are not cheap, however. Exam fee prior to midnight: $102. Exam fee after midnight: $120. They accept Care Credit.

I have a personal story. I took my 9 month old in for acute respiratory distress and dehydration. She had been to our regular vet that afternoon but spiraled quickly after.  I was there from 1140pm to 0345 in the morning and had to leave her there in the oxygen chamber. She truly ad been near death. No doubt it wasn't a fun experience for her but they saved her life. I was able to take her home the next day at 145pm. Both the night and day vets were experienced and knowledgeable and I would take any of my babies there again without hesitation.

I took my 2 month old baby rat in to urgent care because his breathing was raspy and bubbly. They are a very busy clinic so after they stabilized him, I still had to wait maybe another hour and half for a room to finally see the vet myself. (They took Romeo away from me to treat/stabilize him while I had to wait in the waiting room for an agonizing hour or so). I would say that the vet was pretty good, but not very connecting. It seemed more of a get in and get out type of appointment which I really did not find comfortable because I was worried my little baby was going to die and she didn't reassure the best she could have. But she sent me home with antibiotics and an anti-inflammatory and I babied and treated my little guy own my own and he's doing much better. So overall, they are good in what they do, but not in comfort or reasurrance.

They see rats and pretty much every exotic (while I was there, there was a lizard in the oxygen tank next to my rat).  The staff there were incredibly courteous and kind.  They appeared to be very knowledgable about treatment.  My rat unfortunately did not make it (no fault of there's) and I was very surprised by a beautiful card with a rat on it, with a handwritten note by the vet who did his intake.  They also took pawprints on some cardstock and sent them with the card.  I was really touched by this, they are not my normal vet, but still did this for me.

Foster Small Animal Hospital in North Grafton, MA takes exotics 24/7. They're run by Tufts Veterinary School. I've never taken one of my rats there, but I have brought multiple rabbits and a hamster. They have an exotics vet on site during normal hours, and one on call during the night and on weekends. The wait to actually speak to a vet can be long, but they take animals in pretty much as soon as they arrive, so your animal will be in treatment very quickly but you'll likely have to hang out in their waiting area. During normal hours, though, I can't recommend Kensington Bird and Animal Hospital in Berlin, CT enough to anyone in the area. All of their on-staff vets are exotics vets, and they're wonderful, experienced, and not too expensive.
