Looks like kidneys are about done

My 19.5 year old kitty Gizmo was diagnosed with chronic renal disease in the spring.  We tried the subQ fluids and it just did not go well.  Stressed both of us big time and she seemed to be doing fairly well without them.

Unfortunately, she has deteriorated quite a bit over the past couple weeks and over the weekend she started having trouble with her hind legs and she was clearly very dehydrated.  The decrease in food consumption has been pretty gradual and sometimes hard to notice since she never eats all of it anyways.  I took her to the vet Tuesday afternoon and he suspected the kidney disease had progressed or she had developed diabetes.  He gave her a round of subQ fluids and that seemed to help her. He also took some blood and urine for tests. I about choked when they weighed her and her weight was down to 5-11.  I thought she might have lost a little, but I would have never guessed that much. :(

The results came back today and the kidney disease has progressed significantly, but everything else is normal.   The vet said she is basically at the blood levels for end stage renal failure and at this point there really isn't much to do besides keep her comfortable.  Her BUN is 134 and normal is up to 34.  Her creatinine is 8.8 and normal is up to 2.3.  Phosphorus is 16.7.   The urine specific gravity is also a little low.

The vet said that subQ fluids would be an option to help keep her feeling better and maybe give her a little extra time.  I'm not sure if I want to try that again or not.  She said it's important GIzmo eats, whatever it is -- kitty junk food, people food, tuna juice, kitty treats, baby food, whatever.  Nothing she eats can be as bad for her as not eating, so don't worry about trying to get her on a kidney diet or feeding the best food or anything.  Whatever she eats is what's best for her.

Her best guess on prognosis based on her condition and labs is days to weeks.  Once she won't even take people food, that's usually the end and she would only have days left and we would need to consider euthanasia at that point, and maybe even sooner if her life quality deteriorates.  At the vet yesterday, he said he didn't think she was in pain but that she probably just felt lousy.  I think she is some better after the hydration. 

The good news IMO is that she stills seems very interested in food.  She just doesn't eat much when she gets into it.  But she was all over some bites of cheese and ham that I gave her, so I think she still wants to eat.  So I bought a few different types of kitty crack at petco last night and we'll see how she does.  Some of the stuff I've given her before makes her sick (Trader Joe's tuna for cats was an ugly scene!!) She seems to like the kitty treats so I was hoping to find something like the tender vittles they used to sell, but no luck. But I'll try a few things and see if she will eat enough to keep her feeling better enough to drink more which will make her feel better.

Does anyone have experience with renal failure where the lab values were that high?  Is there much to gain in trying to do subQ at home?  I don't want to put either of us through that stress if it will likely only buy a couple more weeks.

If it stresses you both out so much, it's probably not worth the time it'll give you.  When you're only talking days to weeks anyway, to me, that's not the time to be adding extra stress.  Granted, I've never dealt with a kitty in end stage renal failure, so it's just my opinion, given the time the vet seems to think she has left.

You might try some of the meat flavored baby foods for her, since that will give her added moisture to her diet.  :)  I have a friend who feeds her cat baby food, and he adores it.  Of course, he might just be weird, but it's probably worth a shot.

I don't have anything to add, but wanted to say that I'm sorry for your kitty's health.  19,5 is an amazing life, but I know that certainly doesn't ease the pain of losing her.  I hope you can both enjoy whatever time you have together and make the most of the time she has left.

I scheduled her final appointment for the morning.  It's going to be a tough night.

Is that a valid reason to use a sick day??

So sorry. IMO You're doing the right thing. If it is any comfort, when we lived in Manhattan we took our cats to a specialist at Manhattan Cat Specialists who had an emphasis in aging felines. Name something like Arnold Pilney. Anyway, the oldest cat he had treated when we last spoke several years ago was 19. So congratulations! Hope your heart heals soon.
